Assisted Group Living
For Adults With Mental Illness, a Caring Place to Call Home

Often, it may seem that the most vulnerable among us have no place to turn. When people need comfort, care, respect, and dignity as individuals, the answer can often be found at the Foundation for Senior Living.

For those with special needs, the Foundation for Senior Living offers Assisted Group Living. This program provides a caring, homelike environment for adults with mental illness, who require professional supervision and care.

The Assisted Group Living Program operates community-based homes throughout Maricopa County, all of which are State-licensed Behavioral Health Facilities. These homes are designed for adults who have medical and mental health needs.

All group homes have trained staff who provide 24-hour supervision and support, with a behavioral health professional on-call. A clinical team of medical and psychiatric professionals, knowledgeable in the care of adults struggling with medical issues and mental illness, supervise the program.

Designed to promote self-reliance and independence, the Assisted Group Living Program focuses on wellness and the strengths of individuals and families.

Widely regarded as a model of exceptional community care, the program has had great success with its participants, many of whom had histories of failed placements with other types of programs or their families.

Program Goals:

• Maintain Participants in the Least Restrictive Community Setting
• Maximize Independent Living Skills
• Development of Appropriate Behavioral and Social Skills
• Enhance Individual Dignity
• Increase Level of Functioning
• Avoid Rehospitalization

Services Available Include:

• Planned, structured, and Coordinated Therapeutic Activities
• Behavioral Health Management
• Professional Medication Monitoring (available at some sites)
• On-Call Clinical Team
• Transportation for Appointments and Therapeutic Services
• Assistance with Self-Medication Program
• Life Skills Training/Independent Living Program
• Participation in Licensed Adult Day Health Care Program
• Psychosocial Rehabilitation
• Individual/Family/Group Counseling (at designated locations)
