The Pathways Program assists in recognizing possibilities for creative and positive life changes.
Pathways provides personalized, professional services to Seniors and their families, to identify resources, discover care options, and make informed choices.
We know that every person is an individual, with specific strengths and challenges. So we begin by listening carefully and honoring each unique situation.
Pathways Social Workers provide counseling and consultation, information and referral to community resources, and continued support throughout the entire process.
Every person has their own special plan and timing to uncover solutions, restore hope and get back on track. Pathways serves a guide to enhance communication, decision-making, and relationship skills to help restore quality of life.
Special care is taken to address finances, medical issues, support systems, housing, and spiritual concerns for Seniors, and their families. In addition, caregiver issues are strongly considered and options are developed.
Personalized, confidential service can be provided by telephone, at the Foundation for senior Living offices, or in your home Monday through Friday, from 8 am to 5 pm. We offer our services in English and Spanish.
