Description:
Monitors health condition of the participants in the program. Serves as a liaison between health professionals and center staff. Assumes leadership role in medical emergencies, does medical follow-up on participants. Provides health education to staff, participants and their families. Ensures that there is a current health review from participants own physician; maintaining an open flow of communication between the participant’s physician and the center, ensuring that accurate information is related to physician. Actively joins in the interdisciplinary team in care planning; this includes presenting medical information, giving input and following through on nursing outcomes. Writes nursing assessments at admission and prior to care plan. Documents progress notes as needed and every 3 months. Maintains a record of all treatments or medications administered at the Center. Always performs in a professional manner as directed by the code of ethics of the nursing profession
