Description:
Temporary Full time Registered Nurse or Licensed Practical Nurse- Monitors health condition of the participants in the program. Serves as a liaison between health professionals and center staff. Assumes leadership role in medical emergencies. Provides health education to staff, participants and their families. Ensures that there is a current health review from participants own physician; maintaining an open flow of communication between the participant’s physician and the center, ensuring that accurate information is related to physician. Actively joins in the interdisciplinary team in care planning; this includes presenting medical information, giving input and following through on nursing outcomes. Writes nursing assessments at admission and prior to care plan. Documents pr ogress notes as needed and every three months. Maintains a record of all treatments or medications administered at the Center. Always performs in a professional manner as directed by the code of ethics of the nursing profession.
