Description:
Full-time position serving both adult day health centers. Provides outreach by conducting tours, intakes, assessments for potential clients which may include home visits. Completes enrollment process with new clients. Communicates with case managers, family, caregivers and other stakeholders. Coordinates transportation options and funding for new clients. Presents new client profiles to staff upon start. Completes social services assessments, social service notes, and develops social service goals. Facilitates an interdisciplinary care plan conference and invites caregivers/case managers. Responsible for data management and collection related to role. Provides advocacy and support to clients/caregivers. Assist caregivers/clients with community referrals.
Bachelors Degree in Social Work, experience in behavioral health preferred, computer skills in Word and Excel helpful. Must be able to multi task and work in a team environment.
